You
can reach our offices Monday
through Friday from 8:00 am to 5:00 pm and some Saturdays from 8:00
am to 12:00 pm. Dr.
Blazic sees patients in his Fairfield office on Monday, Wednesday,
and Friday mornings and one or two Saturday mornings a month.
He sees patients in his Harrison office on Monday afternoons
and half days on Thursdays and one or two Saturday mornings a month.
We ask that you schedule an initial evaluation/consultation with the
doctor. At that time
questions will be answered, the necessary procedure is determined
and discussed, a short health history is taken and we will schedule
you for a surgical procedure if you so desire.
After surgery, you will usually have one or two
post-operative appointments to check your recovery.
A preoperative
consultation is mandatory for patients undergoing general anesthesia
for surgery.
Please assist us by providing the following information at the time
of your consultation:

Your surgical referral
slip and a current x-ray – preferably a panoramic x-ray not more
than one year old. You
may request that your dentist forward the referral and/or x-ray to our
office. If there is not
enough time, please pick them up and bring them to your consultation
appointment. X-rays are
required at the time of your consultation.
If no current or appropriate x-rays are available, our office
can take them at the time of your consultation. |

A list of medications you
are presently taking as well as a list of any medical drug allergies. |

If you have dental and/or
medical insurance, please bring the insurance card(s) or have written
down the insurance company name, address and phone number.
This will save our office time and unnecessary phone calls
tracking this information down. |
IMPORTANT:
All patients
under the age of 18 MUST be
accompanied by a parent or legal guardian at the consultation and surgery
appointment.
We do experience emergency scheduling of patients involved in accidents
and other acute emergencies. We
do try to keep the schedule on time; however, there are instances when
even the best of planning cannot control your waiting room time.
In case of a long wait or unanticipated emergency, we will be happy
to reschedule you at another time. |
